FProstate-specific antigen is a protein secreted by normal and malignant prostate cells in a man. It acts as a diagnostic marker to detect prostate cancer. However, the PSA is secreted by the non-cancerous and cancerous cells and in conditions like prostatitis and benign prostatic hypertrophy resulting in imprecise results. Despite these factors mentioned above, the PSA test is still considered an important biological marker for the preliminary screening and early detection of prostatic cancer. In most cases, the early stage of the disease does not show any symptoms in men.

A PSA test value greater than 4ng/dl is generally accepted as the standard for further prostate cancer evaluation. However, there is no reference to a higher limit—any increase in blood PSA advocates for further evaluation for prostate cancer. Serum PSA value greater than or equal to 20mg per decilitre is a positive predictive value of metastatic skeletal involvement or the spread of the tumour to the bones.

PSA levels between 0 and 2.5 ng/mL, prostate cancer is highly unlikely
2 PSA levels 2.5 and 4.0 ng/mL is considered to be in the normal range
3 PSA levels 10 ng/mL or higher – High chances of prostate cancer

What does the PSA Test Measure ?
The PSA test is a preliminary screening procedure with a tendency for false-positive and false-negative results. It is also seen in conditions like trauma, infections and benign prostatic hypertrophy(BPH), leading to low specificity. Hence the PSA test is always accompanied by digital rectal examination and Magnetic Resonance Imaging (MRI).
Why should I do the PSA Test ?
Your doctor may advise you a PSA test for prostate enlargement in following conditions:
If you are above 50 years of age and have a family history of prostate cancer.
If you have symptoms that could indicate a prostate condition, such as painful or frequent urination, blood in urine or semen, or pelvic or back pain, you should consult your doctor about getting a PSA test.
How is the PSA or Prostate Specific Antigen Test Done ?
The PSA test is a simple blood test with no need for fasting before the test. The blood sample can be taken by the phlebotomist at a pathology lab or at home. The sample collection takes less than a minute. You will need to avoid having sex or masturbating for 24 hours before your PSA test. How often should PSA Test be Done ?
